Child Immunization Consent Forms Grades 6 & 8

Immunizations will be taking place at Mitchell Middle School for grades 6 & 8 students on October 5, 2017 starting at 1:30 pm. An information letter and Child Immunization Consent Form from Southern Health has been sent home with your grade 6 and/or grade 8 child today.
